Recreation Assistant (part time)
The part time Recreation Assistant supports in the coordination and implementation of the center’s on-going schedule of meaningful recreational activities designed to meet the physical, mental, cultural, spiritual, emotional, and psychosocial interests of each resident in accordance with company policies and procedure as well as local, state, and federal guidelines and regulations. Part time hours will consist of 2 weekdays and weekends.
What You'll Do
- Maintains organized, clean, and safe work area, including activity rooms and common areas.
- Manages time, projects, and records effectively. Attend and actively participate in meetings as requested.
- Ensure all mandatory training is completed on time. Assists with new-hire orientation, training, in-services, and staff meetings as needed.
- Participates in the department’s Quality Assurance and Process Improvement (QAPI) program and assists in resolving any issues identified as part of the QAPI process.
- Reports all instances of suspected abuse or neglect or other concerns to management.
- Recognize abnormal signs and symptoms, which may signify a physical change in any resident’s condition that could require nursing intervention; report the same to charge nurse immediately.
- Maintain confidentiality of all resident information.
- Performs other duties as necessary for the successful operation of the healthcare center.
- Assists all activity functions and services in the department.
- Plans, conducts, and evaluates scheduled activities as directed by the Director of Recreation.
- Assists in planning and implementation of special events and projects.
- Communicates to residents and support staff the daily schedule of planned activities.
- Assist in the completion of MDS assessments, activity assessments, reassessments, progress notes, care plans, and oral histories as directed by the Director of Recreation.
- Accurately record resident attendance, participation in, and response to activities.
- Assist with monthly Resident Council meetings as directed by the Director of Recreation.
- Ensure residents are transported safely and appropriately to and from activities.
- Arrange environment and setup space and equipment for activities; returns equipment to storage for safe keeping after use.
- Provide input and feedback to the Director of Recreation regarding resident progress and program effectiveness.
What You'll Need
- Previous experience as a therapeutic recreation specialist or activities professional in a healthcare setting preferred.
- Exceptional interpersonal skills and ability to effectively interact with diverse group of individuals.
- Ability to communicate effectively verbally and in-writing.
- Ability to work flexible hours, including evenings and weekends.
- Ability to accept and exhibit the responsibility to make sound management decisions.
- Basic computer skills.
Physical Therapist
PHYSICAL THERAPIST
MUST BE A GRADUATE OF AN ACCREDITED PROGRAM AS A PHYSICAL THERAPIST PT, HAVE TAKEN THE NPTE OR ELIGIBLE TO TAKE IT AND HAVE A STATE LICENSE TO PRACTICE AS ONE OR ELIGIBLE TO APPLY FOR ONE.
Duties/Responsibilities:
Assesses the needs of residents with physical therapy needs and matches their needs with services available. Meets with residents regularly according to treatment program schedule, evaluates, and maintains records of residents' progress.
Reports progress to management and family members and meets with them on a regular basis to discuss.
Repeats evaluations on a regular basis to measure progress according to professional guidelines.
Keeps current with professional literature and attends seminars on a regular basis to maintain and update professional skills.
Required Skills/Abilities:
Hearing ability sufficient to evaluate residents' physical therapy needs.
Skill communicating verbally and in writing with residents and family members so that all parties understand the program and the degree of progress being made.

The Central Supply Coordinator assesses the need for and provides supplies and equipment in an efficient
manner, and in accordance with applicable state, federal, and Company guidelines.
ESSENTIAL DUTIES & RESPONSIBILITIES

The Weekend Supervisor maintains nursing unit goals and objectives and is responsible for the supervision of nursing unit(s). Maintenance Assistant
The Maintenance Technician is responsible for the regular care and maintenance of the physical plant and grounds as directed by the Director of Maintenance so that the facility is aesthetically pleasing, properly repaired, and well maintained by following all preventative maintenance schedules and company policies and procedures. The Maintenance Technician will perform the routine duties of the Director of Maintenance in the absence of that person. ESSENTIAL DUTIES & RESPONSIBILITIES GENERAL RESPONSIBILITIES Ensure all mandatory training is completed on time. Maintains accurate documentation in the electronic file keeping system. Maintains an organized, clean, and safe work area, including shops and equipment rooms. Manages time, projects, and records effectively. Maintains a working in-home or mobile phone and provides the number to the center. Assists the Director of Maintenance with quality assurance inspections, HUD inspections if applicable, and preventative maintenance review inspections as required. Reports all instances of suspected abuse or neglect or other concerns to appropriate parties. Performs other duties as necessary for the successful operation of the healthcare center. FACILITY UPKEEP & MAINTENANCE Prioritize and complete all work orders and maintenance requests in accordance with company policies and procedures. Completes work order requests same day when possible but no later than ten working days. Completes emergency repairs in the shortest time possible. Checks and reads all gauges, thermometers, water gauges on boilers and thermometers in coolers and freezers as directed by the Director of Maintenance. JOB DESCRIPTION: MAINTENANCE TECHNICIAN 2 of 3 REVISED 12/2022 Maintains proper curb appeal at all times which includes the absence of trash on the grounds/around dumpsters, clean curbs, trimmed shrubs if applicable, no dead plant material, no weeds in mulch beds or curbs, and clean breezeways. Assists the grounds person with custodial work as necessary. Maintains grounds by mowing and trimming as necessary if directed by the Director of Maintenance. Performs light electrical repairs (appliances, switches, circuits, etc.) Replaces fixtures as needed. Repairs concrete, masonry, roofing, and fencing when necessary. Ensuring that glass, linoleum, carpet, screens, windows, appliances, and locks are properly replaced. Hangs and patches sheet rock, painting, wallpaper, and completes other repairs as necessary. Performs carpentry work (I.e., repairing drawers, shelves, doors, casings, moldings, etc.) Performs all preventative maintenance and completes related documentation in the electronic record keeping system in a timely manner. Monitors and records maintenance supplies and materials. Ensures that supplies and materials are only used for center-related repairs. Works as a team with other maintenance staff and the center’s Environmental Services department to ensure that the center is always survey ready. Knows the location and operation of all water, electrical, and gas cutoffs. LIFE SAFETY & HEALTH Works with the Director of Maintenance, facility Administrator, and center staff to maintain a safe environment for patients, co-workers, and visitors. Maintains current knowledge of codes and regulations. Keeps all doors, walkways, and steps clear of obstacles to permit access and prevent possible injuries. Keeps all doors to maintenance area locked when away from area. Enforces the facility’s safety and health programs. Address all physical hazards immediately. Conducts fire safety and response drills and actively participates in all fire safety in-services. Inspects, operates, and maintains all fire safety equipment and records. Works with Director of Maintenance to ensure facility’s Emergency Operations Plan is maintained and updated regularly as needed. Promotes safety and infection control practices. Practices all established safety standards and precautions. PREREQUISITES, SKILLS, & ABILITIES High school diploma or equivalent.
